Privacy policy
1. Who is responsible?
The data controller for this website is Ward Health Care BV, Volmolenlaan 20 bus A, 3000 Leuven, Belgium, company number BE 0737.704.202, represented by Dr Sam Ward. For questions about your data: info@urofill.be.
2. What data do we process?
- Through the contact form: first name, last name, email address, (optionally) phone number, whether you would like to be contacted for an appointment, and your message.
- Technical data: on every visit, our hosting provider automatically processes data such as your IP address, browser and time, to keep the website secure and available. To prevent misuse of the form, we keep an irreversible (hashed) form of your IP address for no more than one hour when you submit a request.
- Map: if you click "Show on map", your browser loads a map from OpenStreetMap, which receives your IP address. Without that click, no connection is made.
Please do not share medical details through the form. We prefer to discuss your medical situation in person during the consultation.
3. For what purpose and on what basis?
- Answering your question and scheduling a consultation, at your request and with your consent (Art. 6(1)(a) and 6(1)(b) GDPR).
- Securing and running the website, and measuring its use with anonymous counts per page and per referring website, based on our legitimate interest (Art. 6(1)(f) GDPR).
We never use your data for marketing and we do not sell it.
4. How long do we keep it?
Data from the contact form is kept for no more than 12 months after your request has been handled. If you become a patient, the statutory retention periods for medical records apply.
5. Who has access?
Only Dr Sam Ward and the staff handling your request. We also work with processors that provide technical support:
- Cloudflare: website hosting and security, and anonymous visit counts (without cookies or IP addresses);
- Combell (Belgium): sending your form message from our mailbox;
- Google: our email service (Gmail), where your message is received.
Any transfer outside the European Economic Area only takes place with appropriate safeguards, such as the EU-US Data Privacy Framework or the European Commission’s standard contractual clauses.
6. Your rights
You have the right to access, rectify, erase, restrict, object to and port your data, and you can withdraw your consent at any time. Simply email info@urofill.be. We will reply within one month.
Not satisfied? You can lodge a complaint with the Belgian Data Protection Authority, Rue de la Presse 35, 1000 Brussels, dataprotectionauthority.be.
